COA Certificate of Accreditation

Know all persons by these presents:

Pursuant to the provisions of Section 460-a of the Social Services Law and Section 804 of the Not-for Profit Corporation Law, due inquiry and investigation having been made, approval is hereby given to the filing of the annexed certificate of amendment of the certificate of incorporation of Graham’s Gift Children’s Foundation on the condition that the purposes and duration included in the certificate as filed are consistent with the following purposes and duration:

To place out destitute, delinquent, abandoned, neglected, abused or dependent children. The corporation’s authority to place out such children shall terminate on . Furthermore, the duration of the corporation’s adoption purpose shall not be extended without the prior written approval of the New York State Office of Children and Family Services.

Approval is also hereby given to authorize the above-named corporation to change its name to:

New Star Kafala

This approval will, upon filing of the certificate with the Secretary of State, give the corporation the authority to engage in the activities set f0rthh or summarized above for the period of time set forth above.

*New Star Kafala will provide upon request to any who inquire: (1) The number of its adoption placements per year for the prior three calendar years, and the number and percentage of those placements that remain intact, are disrupted, or have been dissolved as of the time the information is provided; (2) the number of parents who apply to adopt on a yearly basis, based on data for the prior three calendar years; and (3) the number of children eligible for adoption and awaiting an adoptive placement referral via the agency or person.
